Telemetry Monitoring: Are Admission Criteria Based on Evidence?

چکیده
monitoring of heart rhythm. Telemetry beds are limited in number and expensive1-3; therefore, their use should be evidence based.4,5 The 2004 American Heart Association (AHA) practice standards for electrocardiographic (ECG) monitoring are often not followed and rigorous criteria for inpatient telemetry admissions are often not systematically used, resulting in overcrowding in emergency departments and inpatient monitored beds1,3,6-9 (Table 1). Monitored beds are often occupied by patients who require only frequent nursing care rather than cardiac monitoring.5,6 Few studies have been done to determine whether inpatient telemetry monitoring is beneficial3 and accurately confirms the initial clinical impression, TelemeTry moniToring: Are Admission CriTeriA BAsed on evidenCe?

When do patients need admission to a telemetry bed?
Non-intensive telemetry units are utilized for monitoring patients at risk for life-threatening dysrhythmias and sudden death. Physicians often use monitored beds for patients who might only require frequent nursing care. When 70% of the top 10 diseases admitted through the emergency department (ED) are clinically indicated for telemetry, hospitals with limited resources will be overwhelmed and...
متن کاملAutomated external cardioversion defibrillation monitoring in cardiac arrest: a randomized trial
BACKGROUND In-hospital cardiac arrest has a poor prognosis despite active electrocardiography monitoring. The initial rhythm of approximately 25% of in-hospital cardiopulmonary resuscitation (CPR) events is pulseless ventricular tachycardia/ventricular fibrillation (VT/VF). Early defibrillation is an independent predictor of survival in CPR events caused by VT/VF. The automated external cardiov...
متن کاملTelemetry bed usage for patients with low-risk chest pain: review of the literature for the clinician.
BACKGROUND Telemetry monitoring in patients with low-risk chest pain is highly utilized, despite the lack of quality data to support its use. STUDY OBJECTIVES To review the medical literature on the utility of telemetry monitoring in patients with low-risk chest pain and to offer evidence-based recommendations to emergency physicians. METHODS A PubMed literature search was performed and lim...
متن کاملIs intensive care monitoring necessary after coil embolization of unruptured intracranial aneurysms?
INTRODUCTION Patients with an unruptured intracranial aneurysm treated with coil embolization are routinely admitted to the intensive care unit (ICU) after the procedure; however, this practice is questionable. The purpose of this study was to determine if routine admission to the ICU is necessary for patients undergoing coil embolization of an unruptured intracranial aneurysm. METHODS We con...
متن کاملEvaluation of telemetry utilization, policy, and outcomes in an inner-city academic medical center.
OBJECTIVE To evaluate the appropriateness and intermediate outcomes of telemetry admissions. METHODS We abstracted demographic and clinical data from records of all new telemetry admissions during a 2-month period. To determine appropriateness, 2 authors classified patients using the American College of Cardiology (ACC) guidelines and our telemetry policy. Other utilization and outcome measur...
